Job summary

Centene Corporation is seeking a Senior Care Manager to support the MHS Health Wisconsin plan. This hybrid role includes approximately 50% field-based visits in the Milwaukee area, focusing on behavioral health and social determinants to deliver high-quality, person-centered care.

Ideal candidates have case management and managed care experience, familiarity with HMO/Medicaid, and state-appropriate BH licensure.

Qualifications

• Requires a Master's degree in Behavioral Health or Social Work or a Degree from an Accredited School of Nursing with BH experience.
• 4-6 years of related experience.
• Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN depending on state contract requirements with BH experience.

Responsibilities

• Develops and continuously assesses ongoing care plans/service plans for high-acuity members via phone or in-home visits.
• Identifies providers, specialists, and community resources needed for care including mental health and social determinant needs.
• Coordinates between member/family and care provider team to ensure services are accessible.
• Maintains a caseload with high complexity and volume.
• Monitors status and outcomes, revises care plans for changes in symptoms or side effects.
• Collaborates with healthcare providers to facilitate timely access to care.
• Documents all member information to ensure regulatory compliance.
• Educates members and families on procedures, options, and benefits, including behavioral health.
