You will monitor residents, document changes, and ensure compliance with state regulations. The role supports shifts from 7:00 a.m. to 7:00 p.m.

or 7:00 p.m. to 7:00 a.m., with PRN options. Requirements include an active LPN license and at least one year of relevant experience.

• Valid Licensed Practical Nurse license is required.

Responsibilities

• Provide direct resident care and supervise residents’ needs per care plans.
• Monitor changes in condition and document in accordance with laws and policies.
• Communicate with Health Services Director and help supervise the care team.
