Job summary

Trustwell Living at Westwood Place is seeking a dedicated Licensed Practical Nurse (LPN) for a full-time role with 3 twelve-hour shifts. You will provide direct resident care and supervision of residents’ needs based on individualized care plans.

Responsibilities include monitoring for changes in condition, administering medications, and collaborating with the Health Services Director to ensure regulatory compliance and high-quality care. A current LPN license and driver’s license are required.

Qualifications

• A passion for seniors and enhancing their quality of life.
• A valid Licensed Practical Nurse (LPN) license; Compact license encouraged; unencumbered LPN license required.
• Minimum of at least one year of successful experience as an LPN in assisted living or long-term care.
• Experience administering medication and treatment plans per doctor’s orders and community policies.
• Knowledge of state regulations pertaining to resident care and being state survey ready.
• Experience as a mandatory reporter and ability to intervene/report/investigate abuse or theft.
• Current driver’s license.

Responsibilities

• Provide direct resident care and supervision of residents’ needs based on individualized care plans.
• Monitor residents for changes in condition and signs/symptoms of illness.
• Communicate and document changes with Health Services Director and care team.
• Assist in supervising the care team.
