• Minimum 1 year of LPN experience in assisted living or long-term care.
• Experience administering medication per doctor orders and community policies.
• Knowledge of state regulations pertaining to resident care and being survey-ready.
• Experience as a mandatory reporter and ability to intervene/report suspected abuse or theft.

Responsibilities

• Provide direct resident care and supervise residents’ needs per care plan.
• Monitor changes in residents’ conditions and document changes.
• Communicate and document concerns with Health Services Director and supervise the care team.
