Qualifications

• Bachelor’s degree or applicable licensure required.
• 2–3 years of case management and care coordination experience.
• Proficiency with electronic health platforms and health information systems.
• Ability to coordinate with staff, clients, and community partners independently.
• Strong organizational and prioritization skills.
• Knowledge of Trauma-Informed Care practices.
• Excellent verbal and written communication skills.
• Familiarity with healthcare delivery systems and community resources.
• Willingness to work in varied client environments (odor, pets, smoke).
• Proficiency in Microsoft Office Suite and internet navigation.
• Valid WA state driver’s license and automobile insurance.

Responsibilities

• Intensive case management and high-touch client support across medical, social, and emotional needs.
• Develop and monitor individualized care plans with 90‑day goal setting.
• Collaborate with hospital discharge planners, physicians, and providers.
• Coordinate transitions and referrals to maintain continuity of care.
• Document in electronic health systems and monitor progress.
