Job summary

Senior Helpers of Rock Hill is seeking a compassionate, relationship-driven Client Care Manager (CCM) to manage the overall client experience within an assigned territory. You will be the primary contact for clients and families, helping convert new clients, retain existing ones, identify changing care needs, and grow services as needed.

The role requires proactive, organized field work, with travel to client homes and care settings, and strong ability to build trusted relationships with seniors

Qualifications

• Bachelor's degree and three years of related professional experience strongly preferred.
• Minimum of two years of experience managing client, patient, customer, or family relationships.
• Previous home care, senior care, healthcare, or related industry experience strongly preferred.
• Exceptional interpersonal skills with the ability to quickly establish trust and rapport with seniors and their families.

Responsibilities

• Serve as the primary relationship manager for assigned clients and their families, providing responsive, proactive communication throughout the client relationship.
• Conduct initial client assessments and develop individualized care plans using LIFE Profile assessment process.
• Follow up with prospective clients and families after inquiries and assessments to help them understand care options and move forward with services.
• Maintain onboarding for new clients, including caregiver introductions, first-day follow-up, and scheduled check-ins in first 30–90 days.
• Conduct regular in-home client visits and reassessments to evaluate changing needs and quality of care.
