Job summary

HomeWell Care Services CA285 in Chula Vista, CA seeks a Part-Time Client Care Manager to oversee client-care quality and support caregivers. This is a care-management role, not direct caregiving, ideal for a CNA or LVN transitioning to coordination and QA.

Approx. 15–20 hours weekly with pay between $27 and $36 per hour, depending on credentials. W-2 position with benefits; travel throughout the service area required.

Qualifications

• 2+ years of senior-care experience preferred.
• Experience with care coordination, supervisory visits, or case management.
• Strong communication and documentation skills.
• Comfort coaching caregivers and addressing performance concerns.

Responsibilities

• Conduct in-home client assessments and reassessments.
• Develop, review, and update Plans of Care.
• Conduct supervisory and quality-assurance visits.
• Follow up with clients and families after new starts of care.
• Monitor client satisfaction and identify concerns early.
• Coach caregivers on Plans of Care and expectations.
• Maintain accurate documentation of assessments and visits.
