Job summary

Position Summary

The Client Care Manager serves as a key link between our clients, families, caregivers, and office team.

The position is responsible for conducting client assessments and supervisory visits, developing and updating Plans of Care, monitoring client satisfaction, supporting caregiver-client matching, coaching caregivers, and identifying changes in client needs.

This is primarily a care-management and quality-assurance position, not a direct-care caregiving position.

Key Responsibilities
  • Conduct in-home new-client assessments and reassessments.
  • Develop, review, and update individualized Plans of Care.
  • Conduct scheduled supervisory and quality-assurance visits.
  • Follow up with clients and families after new starts of care.
  • Monitor client satisfaction and proactively identify concerns that could affect client retention.
  • Observe and identify changes in clients' functional abilities, behavior, safety, or care needs and appropriately communicate concerns.
  • Help determine caregiver skills, personality, availability, and compatibility for appropriate client-caregiver matching.
  • Work closely with the Scheduler/Care Coordinator regarding caregiver assignments and client needs.
  • Coach caregivers regarding client-specific expectations, Plans of Care, professionalism, and performance.
  • Address client and family concerns professionally and promptly.
  • Assist with caregiver performance improvement when concerns are identified.
  • Support new starts of care to ensure a smooth transition for clients, families, and caregivers.
  • Maintain accurate and timely documentation of assessments, supervisory visits, incidents, client concerns, and follow-up activities.
  • Communicate significant concerns to agency leadership and appropriate parties.
  • Maintain professional boundaries, confidentiality, and compliance with HomeWell policies.
  • Participate in regular care-management and operations meetings.
  • Provide a brief weekly report on client-care quality, outstanding concerns, and follow-up needs.
Preferred Qualifications
  • Current California CNA certification or LVN license preferred.
  • Minimum of 2 years of experience working with seniors, preferably in home care, senior living, hospice, home health, skilled nursing, or a related setting.
  • Previous experience with client assessments, care coordination, supervisory visits, caregiver supervision, case management, or client services strongly preferred.
  • Experience supporting individuals living with dementia, Alzheimer's disease, chronic conditions, mobility limitations, or other age-related needs.
  • Strong communication skills and confidence working directly with clients and family members.
  • Ability to coach caregivers and professionally address performance concerns.
  • Strong assessment, documentation, organization, and problem-solving skills.
  • Comfortable working independently in clients' homes and collaborating with an office-based team.
  • Comfortable using electronic care-management and documentation systems.
  • Valid California driver's license, reliable transportation, and ability to travel throughout the service area.
Ideal Candidate

Our ideal Client Care Manager is compassionate but also confident and accountable.

You should be comfortable walking into a client's home, assessing how services are going, listening to family concerns, communicating expectations to caregivers, recognizing changes in client needs, and following an issue through until it is resolved.

We are especially interested in candidates with strong experience in dementia care, senior home care, caregiver supervision, and family communication.

Schedule

Approximately 15–20 hours per week, primarily during weekday business hours.

A typical schedule may include:

Monday: 9:00 AM–1:00 PM – Client/family follow-ups, weekend issue review, Plans of Care, and caregiver concerns

Tuesday: 9:00 AM–2:00 PM – Client assessments, supervisory visits, and starts of care

Wednesday: Normally off/flexible – May occasionally be used for a scheduled assessment or urgent client need

Thursday: 9:00 AM–2:00 PM – Assessments, quality‑assurance visits, caregiver coaching and family follow-ups

Friday: 9:00 AM–1:00 PM – Client satisfaction follow-ups, documentation, and preparation for the following week

The schedule may be adjusted based on client assessments, new starts of care, supervisory visits, and operational needs.

This position is not intended to be a 24/7 on‑call position.

Compensation

$27–$36 per hour, depending on credentials, relevant experience, and demonstrated care‑management capabilities.

General hiring range:
  • Experienced CNA / senior‑care professional: approximately $27–$30/hour
  • Experienced LVN with strong care‑management experience: approximately $31–$36/hour

Mileage reimbursement will be provided for authorized business travel in accordance with company policy and applicable requirements.

What Success Looks Like

The Client Care Manager will be expected to:

  • Complete required assessments and supervisory visits on time.
  • Maintain current and accurate Plans of Care.
  • Follow up promptly after new starts of care.
  • Respond to client/family concerns within one business day.
  • Identify dissatisfied or at‑risk clients before concerns escalated.
  • Document client and caregiver issues accurately.
  • Support strong caregiver‑client matches.
  • Coach caregivers and follow through on performance concerns.
  • Maintain proactive communication with clients and families.
  • Help improve overall client satisfaction, quality of care, and retention.
30-60-90 Day Expectations

First 30 Days – Learn & Assess

Complete orientation and required training, become familiar with HomeWell policies and procedures, review active client Plans of Care, learn documentation and escalation procedures, become familiar with higher‑risk clients, and begin conducting client visits and follow‑ups.

By Day 30, the Client Care Manager should be able to independently conduct routine supervisory visits and document findings appropriately.

Days 31–60 – Take Ownership

Begin managing assessments, reassessments, supervisory visits, family follow‑ups, caregiver coaching, Plan of Care updates, and client concerns independently. Identify clients who may be dissatisfied or at risk of discontinuing services and develop appropriate follow‑up actions with agency leadership.

Days 61–90 – Manage Quality

Assume day‑to‑day ownership of client‑care quality and proactive client follow‑up. Monitor client satisfaction, identify changes in client needs, coach caregivers, support difficult caregiver‑client placements, participate in new starts of care, and provide leadership with a weekly care‑management report. By Day 90, the Client Care Manager should be able to manage routine client‑care quality issues independently while appropriately escalating significant concerns.

The Client Care Manager will provide a brief weekly summary covering:

  • New client assessments
  • New starts of care
  • Supervisory/QA visits completed
  • Plans of Care updated
  • Client/family concerns
  • Caregiver performance concerns
  • At‑risk clients

  • Outstanding follow‑ups
  • Recommended actions for the following week
