Job summary

CAPECO Community Action Program of East Central Oregon is seeking a Case Manager to provide a full range of care management and service authorizations for older adults and people with disabilities. This role covers Hood River, Wasco, Sherman, Wheeler, Gilliam, Umatilla, and Morrow Counties, with primary office work and potential hybrid/remote options after training.

The position requires an associate degree, strong communication and data-entry skills, and experience coordinating in-home services

Qualifications

• Associate degree in related field or equivalent combination of education and experience.
• Experience providing services to older adults and/or people with disabilities.
• Ability to follow complex written and oral instructions and procedures.
• Must have computer skills and data entry software knowledge.
• Able to prepare reports and communicate effectively with clients and providers.

Responsibilities

• Conduct home visits to assess ADLs/IADLs and coordinate in-home services.
• Provide person-centered options counseling and follow-up.
• Develop care plans to support independence and coordinate referrals.
• Coordinate with agencies and maintain regular client contact.
• Maintain client data in state databases and Excel spreadsheets.
