Responsibilities

• Build and maintain relationships with healthcare providers and community organizations.
• Meet with prospective clients and families to assess needs and care preferences.
• Connect families with Care Professionals and resources.
• Conduct quality assurance visits and follow up to ensure care quality.
• Develop referral relationships to support growth and partnerships.
• Represent Home Instead at health fairs and community events.
• Maintain client, referral, and activity data in the CRM accurately.
• Track goals, activities, referrals, and outcomes.
• Communicate effectively with clients, families, and team members.
• Collaborate with office staff to improve client satisfaction and community relations.
• Perform other duties to support clients and Home Instead team.
