Job summary

LifePath in Greenfield, MA is seeking a full-time Home Care Case Manager (35 hrs/week) based in our Greenfield office serving Franklin County and North Quabbin area.

The Case Manager develops supportive relationships with older adults, coordinates Home Care services, assesses needs, and refers clients to resources. Strong communication and documentation skills are required; bilingual abilities qualify for a pay differential after fluency assessment.

Qualifications

• Bachelor's degree preferred and 2-3 years of health or human services experience.
• Licensure in Social Work is a plus.
• Willingness to work with older adults and diverse populations.

Responsibilities

• Develop supportive relationships with older adults and foster independence.
• Evaluate, assess needs, and oversee Home Care services for eligible clients.
• Disseminate information and make referrals; set up and monitor service plans.
• Advocate for clients and collaborate with interdisciplinary team.
