Job summary

Family Service League in Greenport, NY is hiring a full-time Case Manager for our Long Island Sound Senior Care program. You will coordinate health, social and financial services for senior citizens, handling intakes, assessments and care planning.

The role requires an Associates degree (Bachelor's preferred) and at least five years of experience in linking individuals to health-related services. Strong interpersonal and written communication skills are essential.

Qualifications

• Associates Degree required; Bachelor's degree in Human Services, Social Work, Sociology or related field preferred.
• Five years' experience assessing and linking individuals to health-related services and programs.
• Excellent interpersonal and verbal and written communication skills.

Responsibilities

• The Case Manager conducts intakes, assessments, service planning and follow up.
• Develop and facilitate recreational, educational and social activities.
• Provide access and help navigate systems and programs.
• Obtain all proper consent forms to coordinate care.
• The Case Manager works with government and community agencies.
• Ensure clients receive needed services, including provision of transportation to appointments.
• All other duties as assigned.
