Family Service League is seeking a full-time Case Manager who will provide comprehensive care coordination and support to individuals with complex behavioral health, medical, and/or substance use needs. The Case Manager will ensure that all program visit and documentation requirements set forth by the New York State Department of Health (DOH) for the Health Home program are met, while promoting client engagement, wellness, and recovery through person-centered services.

QUALIFICATIONS:
Bachelor's Degree required; master's degree in a health-related field preferred.
Minimum of two years' experience providing direct services to people with Serious Mental Illness, medical conditions, developmental disabilities, and/or substance abuse required.
Excellent interpersonal and verbal and written communication skills are required.
Candidate must possess strong assessment, engagement, organizational and time management skills.
Proficient computer skills, including Microsoft Office required; experience with EHR preferred.
Valid and clean New York State Driver's License required.
PHYSICAL REQUIREMENTS:
This position often requires sitting and working at a computer for extended periods of time, traveling to different FSL offices, community meetings, home visits, etc.
