Job summary

Apple Valley Village Health Care Center in Apple Valley, MN is hiring an Activities Assistant to support transitional care residents. You will conduct interviews, tailor activities, and document plans to enhance comfort and engagement.

The role is full-time, 30 hours weekly, with flexible shifts including weekdays or weekends, days or evenings. Salary ranges from $17 to $22.85 per hour, depending on experience.

Qualifications

• Prior experience working with seniors is preferred
• Demonstrate good time management and organizational skills
• Ability to display enthusiasm and a positive attitude in promoting activities
• Genuine curiosity and enjoyment meeting new people and listening to their stories
• Good communication skills, including ability to give directions clearly

Responsibilities

• Welcome and interview newly admitted transitional care patients
• Learn about patient interests, routines, family, career, hobbies, faith preferences and what is meaningful to them
• Identify activities and opportunities to support comfort, engagement and quality of life during rehabilitation
• Encourage participation while respecting patient preferences, abilities and choices
• Complete required documentation including assessments, observations, care plans and other documentation
• Prioritize new admissions and documentation requirements in a busy setting
• Communicate relevant information with the interdisciplinary care team
• Assist with individual visits, small groups and facility activities
• Document activity attendance and level of involvement
• Represent the facility with courtesy and respect at all times
• Perform other related tasks as needed
