Job summary

SUN Behavioral Delaware seeks a licensed mental health professional to provide comprehensive psychosocial assessments and coordinate admissions for adults with complex mental health and/or substance use needs. The role involves evaluating needs, planning care, and collaborating with a multidisciplinary team to ensure patient safety and timely services.

Responsibilities include conducting interviews, developing treatment plans, coordinating discharge, and maintaining robust clinical documentation

Qualifications

• Master’s degree in a mental health field.
• Current unencumbered license in state of employment (LPC, LMFT, LSW, LISW, LISW-S, LPCC, LPCC-S, LMSW, or LCSW). CPR and de-escalation certification.

Responsibilities

• Complete psychosocial evaluations and develop treatment plans.
• Interview patients, families and others to identify issues and level of disruption.
• Coordinate admissions and urgent detentions with medical staff.
• Facilitate discharge planning and linkage to outpatient services.
• Document all patient interactions and maintain medical records.
• Engage in safety, quality improvement initiatives and CPI-informed crisis management.
