Responsibilities

• Conduct initial pre-screening of clients to determine eligibility for services and appropriateness of case management services.
• Completes intake, assessment, development of service plans, and referrals.
• Triage of emergent situations, phone conversations, on-call responsibilities, and ability to negotiate healthcare and social service systems.
• Provide basic HIV education to patients and support system.
• Maintain accurate program case records and reports to DOHMH.
• Advocate for clients to obtain full range of needed services and coordinate them.
• Promote linkage development and monitor linkage with service providers.
• Ensure community follow-up to engage clients in care and promote appointment adherence.
• Participate in multidisciplinary case conferences and regular caseload supervision.
• Coordinate behavioral health care management in collaborative care.
