Job summary

Qualifications

Responsibilities

• Provide and coordinate behavioral health care management for patients in collaborative care.
• Document encounters in databases and formats required for reimbursement and organizational policies.
• Conduct mental health assessments of patients in the Adult Medicine clinic.
• Provide brief, structured psychotherapy and behavioral activation for depression and anxiety.
• Coordinate treatment options with patient and medical provider; monitor changes in symptoms and medication side effects.
