Job summary

Endeavor Health in Evanston, IL is seeking a Behavioral Health Clinical Case Manager II to provide psychosocial assessments, treatment planning, and discharge coordination for inpatients. You will manage a full caseload of moderate to high complexity patients and collaborate with a multidisciplinary team to improve discharge outcomes.

Required qualifications include a Master’s in Social Work, Illinois licensure (LCSW/LCPC/LMFT or PsyD/PhD), CPR and CPI within 90 days, and at least two years of

Qualifications

• Master's Degree in Social Work or a recognized mental health field.
• Current Illinois licensure with four letters (LCSW/LCPC/LMFT) or PsyD/PhD.
• CPR certification and CPI certification within 90 days of hire.
• Minimum of 2 years in a behavioral health environment.

Responsibilities

• Conduct psychosocial assessments and independently develop, implement, and adjust treatment and discharge plans.
• Manage a full caseload of moderate to high complexity patients with discharge barriers.
• Provide crisis intervention and evidence-based services across settings.
• Lead discharge planning and coordinate transitions of care across inpatient, outpatient, and community services.
• Ensure compliance with regulatory standards and high-quality documentation.
