Job summary

Adapt Integrated Health Care in Roseburg, OR, is seeking an Older Adult Behavioral Health Specialist. The role focuses on cross-system coordination, complex case consultation, and direct services to older adults.

Qualifications

• Bachelor’s Degree in Social Work, Psychology, or related field.
• Must be eligible to register as a QMHA with MHACBO.
• Dual diagnosis experience preferred.
• Must be 21+ and possess a valid driver's license.

Responsibilities

• Concurrent care management and influence on treatment plans for older adults.
• Build and promote community partnerships to support treatment goals.
• Coordinate care across behavioral health, primary care, housing and other services.
• Coordinate focus groups, SWOT analysis, and impact assessments.
• Develop partnerships to provide client resources for older adults.
• Lead multidisciplinary clinical meetings and fidelity strategies.
• Develop coalitions to keep older adults active in the community.
• Increase opportunities for social and civic engagement for older adults.
