Job summary

National Health Foundation (NHF) seeks an Associate Director of Social Services to provide operational leadership for Recuperative Care programs in Los Angeles. Under the Director of Social Services, this role will ensure client-centered services, effective documentation, discharge planning, and program compliance across locations.

The ideal candidate will hold LCSW licensure (MSW preferred), have at least five years of progressive social services experience and three years of supervisory

Qualifications

• Must have Licensure - LCSW.
• Master’s degree in Social Work, Counseling, Public Administration, Human Services, or a related field preferred.
• Minimum of five years of progressively responsible experience in social services, case management, homeless services, healthcare, behavioral health, or a related field.
• Minimum of three years of supervisory or management experience overseeing social services or case management staff.
• Experience with HMIS, electronic client records, case management databases, or similar systems preferred.

Responsibilities

• Provide day-to-day leadership, direction, coaching, and support to Social Services staff.
• Oversee and coordinate social services and discharge planning across Recuperative Care locations.
• Ensure timely, accurate documentation and compliance with program requirements.
• Support housing navigation and linkage to community resources.
• Collaborate with hospitals, community partners, and internal departments to deliver client-centered care.
• Identify training needs and staff development opportunities.
• Monitor program performance and drive quality improvements.
