Position Overview

Select Home Health Services is a clinician-based, locally owned and operated skilled home health care agency in central Indiana. Responsibilities

• Follow care instructions from nurses and therapists.
• Assist with hygiene (bathing, oral care, dressing).
• Record measurements (vital signs, intake/output) accurately.
• Observe and report safety hazards or significant client changes.
• Attend staff meetings, complete required inservices, and participate in orientation.
• Report patient complaints to the RN.
• Maintain confidentiality and adhere to HIPAA and agency policies.
• Follow written and oral instructions.
• Report abnormal findings as observed and per RN care plan.
• Perform delegated non-invasive treatments and document vital signs, height, weight, environmental situations, and client behaviors.

Additional Tasks

• Assist with ambulation, positioning, and turning.
• Transport patients as needed.
• Support personal hygiene and elimination needs.
• Assist with feeding and socialization.
• Reinforce health teachings as planned by RN.

Qualifications

• High school diploma or equivalent preferred.
• Ability to read, write, and follow directions.
• Certified nursing assistant with proof of certification.
• At least one year of full-time experience in direct patient care in an institutional or agency setting within the last five years.
• Positive attitude and ability to work well with patients, families, and staff.
• No findings against the aide related to abuse, neglect, or misappropriation on the nurse aide registry.
