Job summary

  • Assist individuals with ADLs as needed.
  • Participate in shift change reports and relay critical information.

About McIntosh Trail

At McIntosh Trail Community Service Board, we offer individuals experiencing symptoms associated with mental illness, addictive disease and/or developmental disability the hope for optimal functioning and recovery by providing quality behavioral health services and supports.

Summary

Under direct supervision, performs basic patient care and works with patients/clients to develop daily living skills. May take vital signs, under the direction of the Register Nurse. As well as, guards, patrols, or monitors premises, facilities, and other areas to prevent theft, violence, and infractions of rules, Secures and protects facility in case of fires, natural disaster, or other hazardous events.

Responsibilities
  • All new individuals and their property are thoroughly searched. All belongings are documented into Essentia.
  • Orient new individuals to the unit (phone and mealtimes).
  • Monitored individuals' behavior and report new/sudden change in behavior to the LPN.
  • Complete required documentation for services rendered by utilizing the recognizable and acceptable documentation formats for HST/Nursing assessments. Ensure documentation reflects the individual's daily activities (eating, hygiene and sleeping patterns).
  • Perform safety checks per level of security (Q 15min, 1:1 or LOS)
  • Assist individuals with their ADLs as needed.
  • Perform skilled groups for the individuals per Pinewood policy.
  • Ensure premises are safe by monitoring suspicious behavior outside. Report any unusual finding to Director/Charge nurse.
  • Ensure that individuals/family members follow the policy regarding safety (smoking on the premises is not allow).
  • Monitor the Unit/CSC/Temp OBS for illegal weapons, drugs, and contraband.
  • Secure all exits, doors, and windows every 4 hours after 7pm
  • Respond to all alarms and react in a timely manner.
  • Follow the policy on the proper way to search individuals upon admission and how to document their inventory
  • Participate in the morning and evening shift report. Pass all important information to the oncoming shift. (Elopement precautions, jail holds,1:1)
  • Follow the policy regarding break/lunch times.
  • Follow the policy on uniform attire. No long nails, dangling earrings or loose jewelry.
  • Always receive prior approval from Lead Nurse before showing movies, videos, and/or any type of internet access to individuals
  • Refrain from using CELLPHONES on the unit and/or in the presence of individuals (Policy 4106 & 1122)
  • Never leave Post without prior approval from Lead Nurse or Supervisor
  • Complete all Relias training in a timely manner.
  • Work Schedules are to be adhere to. Review Pinewood policy on callouts and shift changes.
  • Attend all job related and educational trainings mandated by and for BHCC / Pinewoods and McIntosh Trail.
  • Maintain the ability to work with multi-cultural and diverse populations.
  • Has knowledge of individual's rights and responsibilities and knowledge to work and interact with individuals on site. Has the ability to know staff and organization Code of Ethics, has the knowledge and ability to follow Confidentiality policies.
  • Request and use leave time appropriately; submit leave request ton a timely basis; request and use proper type of leave in accordance with established rules a policy; provide documentation for use of leave when required.
  • Observes established policies on health, safety, security, and sanitation; notifies proper authorities of circumstances or situations that present potential health hazard.
  • All other duties as directed.
Minimum Qualifications
  • High School diploma or GED, AND six months experience in related field
  • Note: Some positions require a valid Georgia's Driver's License.
Physical Requirements
  • Stand for extended periods of time; push, pull and reach; occasionally bend, sit, stoop, and stretch.
  • Have full range of body motion, including handling and lifting of patients.
  • Have a normal range of hearing and eyesight to record, prepare and communicate appropriate reports; specific vision requirements; close vision, distance vision, color vision, peripheral vision, depth perception and the ability to focus
  • Must be able to lift and carry up to 25/50 lbs.
  • Physical Demands may include: sitting, standing and/or walking, pushing/pulling, handling, reaching, bending, twisting, crawling, kneeling, squatting, climbing (ladders or stairs), overhead reaching.
