Astrya Global Medical Staffing is seeking a Physical Therapist for a 26-week assignment in Seattle, WA. The position offers a 8:00AM - 5:00PM schedule with a 32-hour guaranteed minimum during holiday weeks and potential extension.

Responsibilities include delivering PT services, maintaining the state license, and completing credentialing and compliance documentation. Benefits include comprehensive insurance, 401K matching, and a referral bonus program.

Qualifications

• Maintain active state PT license for Seattle location.
• Complete credentialing documentation prior to assignment start.
• Adhere to facility policies and procedures for patient care delivery.

Responsibilities

• Provide physical therapy services as a Physical Therapist.
• Maintain active state license for the assigned location.
• Complete required credentialing documentation prior to assignment start.
• Complete required compliance and contract QA documentation.
• Work a minimum of 32 guaranteed hours during holiday weeks.
• Commit to a 13 to 26-week assignment duration.
• Work day shift hours as assigned.
• Utilize facility-provided equipment during the assignment.
• Adhere to facility dress code requirements as provided during orientation.
• Follow facility parking procedures as outlined during the first day of orientation.
• Provide a minimum of 30 days notice as required per contract terms.
• Comply with all facility policies and procedures for patient care delivery.
• Adhere to overtime billing requirements as applicable at 1.5 times the standard rate.
