Job summary

Applitrack is seeking a 31N Grant Funded School Social Worker at Madison High School to provide direct social work services, assess needs, and develop intervention plans. You will collaborate with educators and families, coordinate with community resources, and respond to crises to support student well-being.

The role requires MSW with MTSS experience and strong communication skills, working closely with teachers, administrators, and external providers to ensure comprehensive support for all

Qualifications

• MSW from an accredited institution.
• LCSW or equivalent certification preferred.
• Temp/Full School Social Work Approval from the Michigan Dept of Education (or eligible for approval via a SSW-310 form from the university)
• Proven ability to implement effective practices for working with students of all ability levels, including those with special needs
• Experience with MTSS frameworks, including the ability to implement and monitor interventions at various levels (Tier 1, Tier 2, Tier 3).
• Ability to work effectively and collaboratively with students, parents, and staff

Responsibilities

• Provide direct services to children, individually and in groups, to address social, emotional, and behavioral needs.
• Conduct assessments to identify children's social-emotional needs and develop appropriate intervention plans.
• Collaborate with teachers to create and implement behavior management plans for students as needed.
• Facilitate communication between the school and families, providing resources and referrals to community services.
• Respond to and manage crisis situations, providing immediate support and intervention.
• Collaborate with teachers, administrators, and other school personnel to support the holistic development of each child.
• Participate in multidisciplinary team meetings and contribute to the development of individualized education plans (IEPs) when necessary.
• Coordinate with external service providers to ensure that families have access to necessary resources, including mental health services, housing, food security, and more.
