Qualifications

• Current licensure as a PTA in the state of practice.
• One year of PTA experience preferred.
• Complies with all relevant professional standards of practice.
• Current CPR if applicable.
• TB questionnaire, PPD or chest X-ray if applicable.
• Current Health certificate (per contract or state regulation).
• Must meet all federal, state and local requirements.
• Must be at least 18 years of age.

Responsibilities

• Provides physical therapy services, including treatment, implementation, and documentation, under the direction of the Physical Therapist and in accordance with the physician’s plan of care.
