Job summary

CrossWinds Counseling & Wellness seeks a Case Manager (School Contract) to serve as a liaison between students, school staff, families, and community agencies. You will support students' personal and social adjustment to facilitate academic achievement.

Responsibilities include advocating for students, communicating with families and staff, coordinating interventions, and maintaining detailed records. Position requires flexibility to work with school needs and summer responsibilities.

Qualifications

• Associate's degree plus two years' experience in health and human services, or four years' experience in health and human services.
• Bachelor's degree in psychology, social work, or counseling is preferred.
• Must complete Case Management Training and provide certificate within 6 months of hire.
• Computer experience in Microsoft Office programs and basic computer fluency.

Responsibilities

• Serve as a student advocate and respond to social and emotional needs.
• Collaborate with parents, teachers, administrators, and supporting agencies.
• Coordinate with CrossWinds staff and school district on interventions and timelines.
• Maintain accurate records and submit progress notes, treatment plans, and reports timely.
