Job summary

Nexus Family Healing is seeking a School Case Manager to support students and families in the Devil's Lake, ND area. The role is a 12-month, full-time assignment, primarily on-site at a local school, with a salary range of $48,000–$54,000.

The position requires a bachelor’s degree in social work (or related field), 1–3 years of relevant experience, and a preference for North Dakota licensure. Eligible benefits include health insurance, PTO, holidays, 401K, and more, with Nexus‑PATH team support.

Join our Nexus‑PATH team as we support our school case management services to the Devil's Lake, ND area schools.

Responsibilities
  • Provide comprehensive coordination of all tasks required to meet the treatment needs and best interests of service recipients consistent with regulatory agencies (i.e., DHS, COA)
  • Comply with local office procedure for referral and service delivery
  • Complete a systemic assessment of the needs of the child and family
  • Establish service parameters with family or legal custodian as child’s needs dictate
  • Provide complete and required documentation, within established timelines, relating to case management and treatment planning
  • Implement treatment services to meet the needs and goals of the children and families utilizing quality case management practice to provide for the best interest of the child and family
  • Provide direction to other team members when applicable and make recommendations for discharge, finalization and permanency
  • Promote the needs of youth and families to ensure successful outcomes
  • Advocate for the development and maintenance of funding and resources to meet youth and family needs
  • Develop and maintain community contacts to promote Nexus‑PATH and collaborative efforts
  • Provide crisis intervention as needed to maintain safety and provide for the best interest of the child and family
  • Integrate trauma‑informed care principles into practice by sharing knowledge and understanding of trauma with the child, family and relevant team members
  • Utilize and advocate for the use of trauma‑sensitive strategies to support the youth and their family
  • Routinely screen for trauma exposure and related symptoms and demonstrate competence in using screening tool results to enhance service provision
  • Routinely make resources available and share knowledge with foster families and providers on trauma exposure, its impact, and trauma‑focused treatment
  • Engage in efforts to strengthen the resilience and protective factors of children and families impacted by and vulnerable to trauma
  • Emphasize continuity of care and collaboration across child‑service systems
  • Understand potential for secondary traumatic stress and practice self‑care strategies to mitigate these effects
  • Meet standards for productivity and caseload size while maintaining appropriate documentation and files to support the services that are provided to families
  • Participate in staff and supervision meetings and required Nexus‑PATH in‑service meetings
  • Recognize and value cultural differences in all aspects of work and service delivery
  • Ensure confidentiality of all treatment and services information
Qualifications
  • Bachelor’s degree in social work preferred; will consider bachelor’s degree in a human services related field
  • 1–3 years of work experience serving children and families
  • Current North Dakota social work licensure preferred
  • Experience in family‑based service strongly preferred
  • Valid driver’s license required. Must meet state regulating agency driving requirements, maintain auto insurance and pass corporate insurance eligibility standards
  • Must successfully pass a DHS/DCFS/CJIS/DHHS/ODHS or other federal and state criminal background check
  • Must have the ability to build strong relationships with diverse individuals or communities and understand their unique needs and challenges
  • Must be open to meeting the cultural needs of individuals or communities, considering factors such as language, traditions, and values
Physical Demands
  • Must be able to assist staff in physical interventions and restraints of clients
  • Must be able to lift up to 50 pounds unassisted and up to 100 pounds with assistance
  • Must be able to stand and walk for extended periods of time (2‑4 hours)
  • Must be able to twist, bend and stretch in a manner conducive to the execution of daily activities within a residential treatment facility
  • All client‑facing Nexus employees must successfully participate and complete a Handle With Care behavior intervention method exam and training
