AltaPointe Health Systems seeks a School Based Therapist to deliver mental health services in the public school system, including individual, group, family, and play therapy, plus outreach to families.

You will conduct assessments, develop treatment plans based on CANS, coordinate with the treatment team, and ensure documentation, ethics, and compliance with AltaPointe policies while maintaining required productivity.

Qualifications

• Master’s degree in a mental health discipline.
• Provides individual, group, family and play therapy as part of services.
• Maintains productivity with at least 110 billable hours monthly as per AltaPointe policy.
• Follows AltaPointe policies and procedures.
• Participates in Performance Improvement activities and AltaPointe committees as required.
• Maintains confidentiality and ethical standards in all interactions.

Responsabilités

• Provides administrative, clinical and consultative mental health services in one or more school settings.
• Conducts clinical assessments and supports DSM-IV-TR diagnoses.
• Forms treatment plans based on CANS with consumer and family/guardian input.
• Delivers individual, group, family and play therapy.
• Provides outreach services to consumers and families.
• Recognizes crisis states and deescala tion or resolves situations.
• Collaborates with treatment teams, school personnel and external sources.
• Assesses discharge needs and completes treatment plan reviews.
• Documents clearly per AltaPointe policy and required timelines.
• Ensures case disposition, closings, follow-ups and referrals.
• Maintains required licensure and CEU compliance.
