Job summary

AltaPointe Health in Alabama seeks a licensed Therapist for its In Home division to conduct clinical assessments, develop individualized treatment plans, and provide therapy (individual, group, play, and family) in community settings.

You will collaborate with families, coordinate with staff and outside resources, document diligently, and participate in performance improvement activities while maintaining ethical standards.

Qualifications

• Master's degree required in a mental health discipline.
• At least 1 year post-master experience in child/adolescent or family therapy.
• Certification in-home intervention and case management within 6 months of employment.
• Valid driver's license and clean driving record.

Responsibilities

• Provide clinical assessments and formulate treatment plans with consumers and families.
• Deliver individual, group, play and family therapy according to the treatment plan.
• Provide outreach services in community settings and collaborate with staff and external sources.
• Assist with discharge planning and maintain timely documentation per policy.
• Model ethical practice and participate in supervision and performance improvement activities.
