Job summary

MVHC is inviting applications for a Licensed Social Worker (LSW) or Licensed Independent Social Worker (LISW) to join the Foxfire School-Based Health Team on an outpatient basis. The role collaborates with physicians, nurses, and other social workers to support patients in a primary care setting.

The position emphasizes patient advocacy, culturally sensitive care, and psychosocial support, with full-time hours and a comprehensive benefits package. Epic EMR proficiency is preferred.

Qualifications

• Bachelor's degree in social work required.
• 2 years of experience as a social worker in a primary care or acute care setting preferred.
• Excellent interpersonal and written communication skills required.

Responsibilities

• Provide social work interventions to remove psychosocial barriers to health in primary care and mental health departments.
• Collaborate with healthcare team to move the patient toward self-management.
• Advocate for and empower patients to maintain control over their health and wellness.
• Triaging and referring patients to specialty mental health care when appropriate.
• Document interventions in patients’ medical records within 48 hours of service.
