Qualifications

• Must release Social Security Number to be eligible for AHCCCS-Provider ID Number.
• Arizona IVP fingerprint clearance card required.
• Proof of immunity to rubeola (measles) and rubella (German measles) or proof of MMR immunization.
• Valid Arizona Driver's license required within 30 days of hire.
• MV Report showing allowable points against Driver’s License.

Responsibilities
