Family of Kidz in Central Suffolk County, NY is seeking a licensed Physical Therapist to design and implement therapeutic interventions for children on your caseload within school district placements.

You will administer evaluations, coordinate with lead supervisors, and maintain attendance and billing logs while collaborating with families and school teams. This role offers full-time or fee-for-service options and a sign-on bonus with active cases.

Qualifications

• Graduate from an accredited university with a degree in Physical Therapy.
• Current New York State Physical Therapy License.
• Bilingual is a plus.

Responsibilities

• Design and implement creative individual and/or group therapeutic interventions for all children on your caseload.
• Administer and write up physical therapy evaluations.
• Ongoing communication with lead supervisors & coordinators, specifically related to case management.
• Maintain professionalism with families, school teams, and other professionals.
• Maintain accurate and timely attendance and billing logs.
• Create and maintain a schedule with assigned individual families/school personnel.
• Collect data on each IEP goal and reports on progress quarterly.
• Provide input and attend/teleconference initial CSE meetings and/or annual reviews.
