Job summary

Family of Kidz is seeking a Physical Therapist to support school-based placements in Central Suffolk County, NY. The role offers a sign-on bonus and a competitive hourly rate with opportunities for full-time or fee-for-service arrangements.

You will design therapies for children on your caseload, perform evaluations, and collaborate with school teams and supervisors to track progress and ensure compliance with IEP goals.

Qualifications

• Graduate from an accredited university with a degree in Physical Therapy.
• Current New York State Physical Therapy License.
• Bilingual is a plus.
• Passionate to work with children and their families.

Responsibilities

• Design and implement creative individual and/or group therapeutic interventions for all children on your caseload.
• Administer and write up physical therapy evaluations.
• Ongoing communication with lead supervisors & coordinators, specifically related to case management.
• Maintain professionalism with families, school teams, and other professionals.
• Maintain accurate and timely attendance and billing logs.
• Create and maintain a schedule with assigned individual families/school personnel.
• Collect data on each IEP goal and reports on progress quarterly.
• Provide input and attend/teleconference initial CSE meetings and/or annual reviews.
