Job summary

Laclinicahealth is seeking a School Based Pediatric/Family Nurse Practitioner to work in Medford, OR, at school-based sites for 5 hours per day, Monday through Friday. The role focuses on primary medical and behavioral health care for children, supporting their emotional and social needs, and collaborating with internal and external primary care providers.

The position aims to keep children in school and provide accessible care when barriers exist, with licensure and DEA/CPR credentials required.

Qualifications

• Graduate from an accredited program with board certification or eligibility.
• Experience with low-income families and migrant/community health preferred.
• Active Oregon license; CPR certification and active DEA license.

Responsibilities

• Provide primary medical care to children in school-based sites.
• Support the emotional and social needs of youth.
• Collaborate with internal multi-disciplinary teams and external primary care providers.
• Maintain continuity of care and facilitate referrals to appropriate services.
• Keep children in school and address barriers to accessing care.
