Job summary

Cumberland Family Medical Center (CFMC) seeks a Nurse Practitioner with school-based primary care experience to provide comprehensive health services to students in designated schools. The APRN will diagnose illnesses, review immunizations, and develop care plans in collaboration with school staff and families.

The role requires MSN, current RN and APRN licenses in Kentucky, and national pediatric NP or family NP certification.

Qualifications

Experience with children/adolescents in school or community health.
Experience providing comprehensive primary care including diagnosis and management of illnesses.
Experience collaborating with interdisciplinary teams.

Responsibilities

Conduct comprehensive health assessments of students, including physical exams, histories, and immunization reviews.
Diagnose and treat common acute and chronic illnesses and injuries within scope.
Develop individualized healthcare plans for students with chronic conditions (asthma, diabetes, allergies).
Administer medications and immunizations per established protocols.
Provide health education and counseling to students, families, and school staff.
Collaborate with school nurses, counselors, and other staff to address health concerns.
Make referrals to specialists or other healthcare providers as needed.
Maintain accurate and confidential student health records.
Participate in school health programs and health fairs.
Stay current with pediatric healthcare advancements and guidelines.
Comply with laws and regulations related to school healthcare practice.
Collaborate with Cumberland Family Medical Center to ensure continuity of care.
Other duties as assigned.
