Job summary

Advocate Health is partnering with Maine West High School District 207 to expand its school-based health center. We seek a dedicated Advanced Practice Registered Nurse (APRN) to provide high-quality, patient-centered primary care within a school setting.

This part-time (.6 FTE) role supports adolescent health, chronic and acute care, and preventive services, working closely with educators and families to improve access to care and student wellness.

Qualifications

• Licensed APRN in Illinois
• Board certification in Family Medicine or Pediatric Primary Care preferred
• Experience in primary care, pediatrics, or school-based health preferred
• Strong communication and collaboration skills
• Passion for working with adolescents and underserved communities

Responsibilities

• Deliver comprehensive primary care services (acute, chronic, preventive)
• Conduct health assessments and treatment planning for adolescents
• Promote health education, wellness, disease prevention
• Collaborate with school staff, families, and healthcare team to coordinate care
• Maintain accurate medical documentation in EMR
• Participate in initiatives to expand school-based health services
