Job summary

Greeley-Evans School District 6 is seeking a Community Schools Nurse Case Manager to support health services and student well-being. The role partners with families, healthcare providers, and school leaders to remove health-related barriers to learning and success.

The position requires a Nursing degree, CO licensure eligibility for school practice, and CPR/First Aid certification, with travel between district sites and community partners. Comprehensive benefits apply.

Qualifications

• Requires Bachelor's degree in Nursing and CO nursing/practice license eligibility for school nursing.
• American Heart Association First Aid and CPR certification required.
• Valid CO driver’s license and ability to travel between school and community sites.
• Eligibility for Instructor Certification in CPR and First Aid.

Responsibilities

• Assess health needs and create care plans for students with chronic or complex conditions.
• Provide direct nursing care during health events and train staff on medications and procedures.
• Coordinate services and connect students to doctors, mental health providers and local resources.
• Collaborate with families and school staff, including home visits when needed, to ensure student safety.
• Track data and maintain confidential health records to support program improvement.
