Job summary

Pediatric Developmental Services (PDS) in Minnesota is seeking a School Based LPN to support student health across local districts. The role performs nursing assessments, administers medications, and helps develop individualized healthcare plans (IHPs) and emergency action plans.

The successful candidate will communicate with families, physicians, and district nursing leadership, maintain accurate health records, and respond to medical emergencies in coordination with EMS as needed.

Qualifications

• LPN nursing license required.
• CPR Certification required.

Responsibilities

• Perform nursing assessments and clinical judgment for student health concerns
• Administer medications, treatments, and manage chronic conditions according to care plans.
• Develop, update, and oversee individualized healthcare plans (IHPs) and emergency action plans.
• Communicate with families, physicians, and district nursing leadership regarding student health needs.
• Maintain accurate health records, documentation, and compliance with state and district guidelines.
• Respond to medical emergencies and coordinate care with EMS when needed.
