Job summary

Meridian Health Services is seeking a full-time School Based Licensed Practical Nurse (LPN) for our school-based clinics in Richmond, IN. This role supports a year-round calendar and may require travel for coverage at other locations during breaks.

Responsibilities include medication management, MAR oversight, patient assessment, health screenings, and education for students and caregivers, with collaboration across medical staff and schools to ensure integrated care.

Qualifications

• Licensed Practical Nurse (LPN) in good standing with Indiana state license.
• Two (2) years nursing experience required.
• Valid driver's license and reliable transportation.

Responsibilities

• Respond to medication questions and consult with medical staff for changes to regimens.
• Oversee MAR documentation and provide corrections as needed.
• Document in school-based records and MHS electronic system.
• Phone in prescription orders as directed by medical staff.
• Review lab/test results and report concerns to medical staff.
• Obtain medical histories and perform vital signs and point-of-care testing.
• Provide health screenings and direct students to integrated medical care services.
• Administer injectable medications as ordered.
• Provide educational groups and one-on-one teaching about medications and health issues.
• Complete clinical documents for review by medical staff.
• Participate in treatment planning reviews and modifications.
• Assist with health records maintenance, including immunizations reporting.
• Serve as liaison with students/caregivers, physicians, pharmacies, and providers.
• Complete additional documentation for billing purposes; travel up to 10% for training/coverage.

Key Responsibilities of a School Based LPN

• Responds to requests from studentscaregiversclients concerning the effectiveness of medications side effects special precautions or problems with medication and consults with medical staff as appropriate to provide directions on changesmodifications to the prescribed medication regimens
• Oversee documentation in Medication Administration Record MAR and provide corrections as necessary
• Document as appropriate in the school based record and the MHS electronic system if a MHS patient visit takes place
• Phone in prescription orders to pharmacies as directed by medical staff
• Review lab and test results reporting concerns and abnormal results to medical staff
• Obtain medical histories and do physical assessments as directed by medical staff Including vital signs and other point of care testing
• Provide health screenings as needed and direct studentsclients to appropriate integrated medical care services
• Administer injectable medications as ordered by medical staff
• Provide as needed educational groups and one on one teaching to studentstheir caregivers and clients concerning their medications diagnosis and health related issues
• Complete required clinical documents for reviewapproval by a member of the medical staff
• Participate in reviews and modifications of studentsclients services during treatment planning
• Assist with maintenance of student health records providing annual screenings testing and follow up on immunizations through the Indiana Department of Education
• Function as a liaison with studentscaregivers with emergency room physician offices pharmacies and the MHS Provider if a patient to clarify medication order obtain proper prescriptions labs testing and referrals and obtain appointments as needed
• Complete other documentation to ensure accurate and timely information for client and third party billing
• Up to 10 travel for training and coverage at other locations as needed

We are seeking School Based LPN candidates with the following qualities

• Must be a non probationary Licensed Practical Nurse who has successfully completed an Accredited Nursing Program and is currently licensed by the State of Indiana
• Valid drivers license proof of auto insurance reliable transportation and a driving record meeting Meridians driving policy
• Two 2 years nursing experience required
