Job summary

Family Health Centers at NYU Langone is seeking a Program Supervisor to lead school-based health initiatives that address social determinants of health for students and families. You will integrate community health workers and health educators into school clinics across elementary and high schools, coordinating with clinical and school leadership.

Qualifications

• BA is required; Masters preferred but not required.
• Two years of direct health care or case management experience; at least one year in program supervision.

Responsibilities

• Develop workflows for community health workers and health educators in school-based centers.
• Build relationships with school staff and partner organizations to integrate CHWs.
• Assess school resources to plan services and protocols.
• Provide supervision to community health workers.
• Refer families and students in need to CHWs.
• Collaborate with data team to ensure needed data fields exist.
• Document outreach, screening, and case management activities.
• Screen families for social determinants of health and navigate to eligible services.
• Support staff in screening for benefits and assisting with applications.
• Use data to improve program and track KPIs.
• Plan and facilitate staff meetings; provide staff development.
• Develop culturally competent outreach materials with partners.
• Maintain referrals and navigation for 1115 waiver eligibility.
