Job summary

NYU Langone Health's Family Health Centers in Brooklyn is seeking a Health Educator to deliver sexual and reproductive health education to adolescents within School-Based Health Centers.

The role includes rapid HIV counseling, risk reduction counseling, and linking youth to medical, behavioral health, and social services. You will conduct SDOH screenings and support enrollment in health coverage programs.

Qualifications

• To qualify you must have a HS Diploma.
• Prior work experience with community health outreach. Health related background.
• Bachelor's degree (preferred).
• At least two years providing services to young adults. Good interpersonal skills.
• Workshop facilitation experience.
• Interest in public health/sexual health education.

Responsibilities

• Provide sexual and reproductive health education to adolescents within Family Health Centers at NYU Langone School-Based Health Centers.
• Conduct rapid HIV counseling and testing services and complete all required documentation in accordance with program and regulatory requirements.
• Provide individualized risk reduction counseling to adolescents and connect youth to appropriate medical, behavioral health, and social support services.
• Conduct Social Determinants of Health (SDOH) screenings with youth and facilitate connection to appropriate resources and supportive services based on identified needs.
• Promote awareness and utilization of School-Based Health Center services through outreach, relationship-building with students, School-Based Health Center tours and other enrollment activities to increase student engagement and access to care.
• Assist eligible patients with enrollment in the Family Planning Benefit Program and other applicable health coverage programs as needed.
• Participate in SBHC morning huddles, care team meetings, and other interdisciplinary meetings as required.
• Provide evidence based sexual health workshops in high school classrooms.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $49,200.60 - $51,000.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
