Four Rivers Community Health Center is seeking a School-Based Community Health Liaison (LPN/MA) to connect students and families with health services via school partnerships in Rolla, MO. The role emphasizes care coordination and community outreach.

The position involves coordinating clinics, assisting with insurance applications, and promoting health resources in collaboration with the district and clinic to improve population health outcomes.

Qualifications

• LPN or MA required; experience with public schools, social services, or public health preferred.

Responsibilities

• Establishes trusting relationships and interacts professionally with district staff, partners, healthcare providers, and families.
• Coordinates obtainment of pre-working consent and registration forms and enters information into patient charts.
• Schedules, coordinates, and attends school-based and community events to improve health access and outcomes.
• Schedules and conducts school-based appointments such as dental exams, well-child checks, sports physicals, immunizations, etc.
• Provides follow-up and referrals after events, coordinating with treatment teams for care coordination.
• Assists families in accessing health-related services, evaluating eligibility for programs, and ensuring enrollment continuity.
• Utilizes EHR, Excel, Word and other software to format documents and materials.
