Job summary

Adult & Child Health seeks a School Based Clinical Evaluator in Indianapolis, IN to conduct initial assessments for students seeking services and establish treatment plans and provisional diagnoses within school-based programs. You may provide brief therapy sessions and safety planning, and, with approval, offer supervision to trainees.

Responsibilities include coordinating referrals, maintaining documentation, participating in peer reviews, and reporting to leadership.

Qualifications

• Master’s degree in Social Work, Psychology, Counseling, or related field required.
• Unrestricted licensure for location and area of practice (e.g., LP, PLP, LPC, PLPC, LCSW, LMSW, PLMFT) required.

• Conduct screenings and assessments to determine service eligibility for students.
• Establish treatment plans and provisional diagnoses.
• Make referral recommendations within internal programs.
