Weekdays, 1st Shift! Full and Part-Time.

This position provides intensive case management to youth identified as at risk within designated schools in the community.

Essential Job Functions
  1. Provide intensive case management.
  2. Conduct case management meetings with parents/caregivers, school personnel, law enforcement, other stakeholders, and youth as appropriate.
  3. Perform intake interviews with youth and families, needs assessments, and individual service planning; obtain and maintain necessary releases of information.
  4. Maintain accurate documentation of meetings, releases, communications, referrals, and other paperwork in a timely manner per Brien Center standards.
  5. Conduct family meetings to ensure good communication between school, family, and Brien Center; make monthly phone calls to families to monitor progress.
  6. Meet with youth at least once a week or as needed, teaching skills such as self-regulation and executive functioning.
  7. Provide advocacy and referrals to appropriate resources for youth and families.
  8. Be available during school hours with some flexibility for late afternoon and evening activities.
  9. Provide 24/7 crisis on-call support for youth and families in the program.
  10. Collaborate with Brien Center clinicians and community clinicians working with youth and families.
  11. Plan and participate in family events to foster positive engagement.
  12. Attend community meetings with clinical or state agencies involved with families.
  13. Attend mandatory trainings and other relevant professional development sessions (e.g., ACRA, Motivational Interviewing, Harm Reduction, etc.).
  14. Collaborate on substance use assessments and treatment planning with clinicians.
  15. Participate in staff meetings, supervision, and required trainings; keep supervisor informed and update collateral services when necessary.
  16. Report incidents involving accidents, injuries, or unusual events.
  17. Be available to transport youth as needed.
Qualifications
  • Bachelor’s degree in a related field.
  • Experience working with adolescents or ability to establish effective relationships with them.
  • Training or experience in substance abuse, counseling, or education.
