This position provides intensive case management to youth identified as at risk within designated schools in the community.
Essential Job Functions
- Provide intensive case management.
- Conduct case management meetings with parents/caregivers, school personnel, law enforcement, other stakeholders, and youth as appropriate.
- Perform intake interviews with youth and families, needs assessments, and individual service planning; obtain and maintain necessary releases of information.
- Maintain accurate documentation of meetings, releases, communications, referrals, and other paperwork in a timely manner per Brien Center standards.
- Conduct family meetings to ensure good communication between school, family, and Brien Center; make monthly phone calls to families to monitor progress.
- Meet with youth at least once a week or as needed, teaching skills such as self-regulation and executive functioning.
- Provide advocacy and referrals to appropriate resources for youth and families.
- Be available during school hours with some flexibility for late afternoon and evening activities.
- Provide 24/7 crisis on-call support for youth and families in the program.
- Collaborate with Brien Center clinicians and community clinicians working with youth and families.
- Plan and participate in family events to foster positive engagement.
- Attend community meetings with clinical or state agencies involved with families.
- Attend mandatory trainings and other relevant professional development sessions (e.g., ACRA, Motivational Interviewing, Harm Reduction, etc.).
- Collaborate on substance use assessments and treatment planning with clinicians.
- Participate in staff meetings, supervision, and required trainings; keep supervisor informed and update collateral services when necessary.
- Report incidents involving accidents, injuries, or unusual events.
- Be available to transport youth as needed.
Qualifications
- Bachelor’s degree in a related field.
- Experience working with adolescents or ability to establish effective relationships with them.
- Training or experience in substance abuse, counseling, or education.
