Weekdays, 1st Shift! Full and Part-Time
This position provides intensive case management to youth identified as at risk within designated schools in the community.
Essential Job Functions
- Provide intensive case management.
- Conduct case management meetings with parents/caregivers, school personnel, law enforcement, other stakeholders, and youth as appropriate.
- Perform intake interviews with youth and families, conduct needs assessments, and develop individual service plans, including obtaining and maintaining appropriate releases of information.
- Maintain proper and complete documentation of meetings, releases, communications, referrals, and other essential paperwork in a timely manner as per Brien Center standards.
- Conduct family meetings to ensure effective communication between school, family, and the Brien Center. Make monthly phone calls to families to monitor progress.
- Meet with youth at least once a week or as needed, teaching skills such as self-regulation and executive functioning.
- Provide advocacy and make referrals to appropriate resources to assist youth and families.
- Be available during school hours with some flexibility for late afternoon and evening activities.
- Provide 24/7 crisis on-call support for youth and families within the program.
- Collaborate with Brien Center clinicians and community clinicians working with youth and families.
- Identify and plan family events to foster positive family engagement.
- Attend community meetings with clinical or state agencies involved with families (e.g., DCF, DMH).
- Attend mandatory Brien Center trainings and other relevant training sessions such as ACRA, Motivational Interviewing, Harm Reduction, and others.
- Collaborate with clinicians on substance use assessments and treatment.
- Participate in staff meetings, supervision, and required trainings.
- Keep supervisors informed and update collateral services as necessary.
- Report incidents involving accidents, injuries, and unusual events.
- Be available to transport youth as needed.
Qualifications
- Bachelor's degree in a related field.
- Experience working with adolescents or the ability to establish and maintain effective relationships with them.
- Training and/or experience in substance abuse, counseling, or education.
