Job summary

Elizabeth Layton Center (ELC) in Paola, KS, seeks a School-Based Children's Case Manager to join the CBS team. Primary duties include treatment planning, client education, and collaboration with school staff and community agencies.

Qualifications

  • Bachelor’s degree or equivalent experience to meet Kansas licensing requirements.
  • Familiarity with mental health theory and practice.
  • Clean driving record.
  • Must pass security checks required by Medicaid regulations.

Responsibilities

  • Participates in treatment planning based on best practice theories and applies interventions.
  • Develops personal plan/goal sheet in conjunction with consumer’s needs.
  • Obtains location and process of acquiring resources; coordinates with area agencies.
  • Direct involvement in planning and advocacy in Education/Vocation, Residential, Financial, Health, Leisure, and Social Supports.
  • Educates family in symptomatology, relapse indicators, and crisis handling.
  • Provides services to identified ELC clients and youth identified by school staff.
  • Educates school staff regarding mental health signs, symptoms, and treatment strategies.
  • Completes required paperwork and reports in a timely manner.
  • Works with other mental health center staff and maintains transportation to activities.

Job Summary:

The School-Based Case Manager will provide case management/therapeutic services to youth with a severe emotional disturbance (SED). In coordination with therapeutic staff, the incumbent will assist youth and families by providing information, education, and training to build knowledge, develop skills, and change attitudes with a goal of increasing the consumer’s independence, productivity, self-determination, integration, and inclusion in the community. Most of the Case Manager’s time will be spent providing services to youth in a school setting. Services may also be provided in the family home, program site or other community settings, with minimal time spent at the Center. This position reports directly to the CBS Coordinator or designee.

Major Job Responsibilities:
  • Participates in treatment planning based on best practice theories and applies interventions
  • Development of personal plan/goal sheet in conjunction with consumer's needs
  • Obtains location and the process of acquiring resources; coordinates with area agencies
  • Direct involvement in planning and advocacy in areas of: Education/Vocation, Residential, Financial, Health , Leisure, and Social Supports
  • Education to family in symptomatology, awareness of relapse indicators and handling of crisis
  • Provision of services to identified ELC clients as well as youth identified by school staff
  • Education to school staff regarding mental health signs, symptoms, and treatment strategies.
  • Assists in collection of program data and provides reports as necessary to immediate supervisor
  • Works in conjunction with other mental health center staff
  • Must provide transportation to and from various program activities
    (must maintain a good driving record)
  • Meets direct service expectations of 47% of time worked
  • Works evening hours each week unless direct service and client needs are met
  • Meets deadlines for daily paperwork and reports
  • Provides quality documentation
  • Complete other duties as assigned by supervisor or designee
  • Completes all required trainings as assigned
  • Complies with ELC policies and procedures
Requirements
Education and/or Experience:
  • Bachelor’s degree or equivalent experience necessary to meet State of Kansas licensing regulations
  • Possesses the minimum qualifications as documented in the Kansas Case Management Program Standards
  • Familiarity with mental health theory and practice
  • Clean driving record
  • Incumbent must pass a security and checks as required by Medicaid Regulations
Knowledge, Skills, and Abilities:
  • Relates a positive approach to complex mental health issues
  • Excellent communication skills in high stress environments
  • Ability to communicate effectively orally and in writing
  • Ability to establish effective working relationships with staff, community agencies, and consumers
  • Ability to be sensitive to cultural and ethnic differences
  • Ability to safely perform major job responsibilities, with or without reasonable accommodations
