The Children's Case Manager will provide case management/therapeutic services to youth with severe emotional disturbance (SED). In coordination with therapeutic staff, the Case Manager will assist youth and families by providing information, education, and training to build knowledge, develop skills, and change attitudes with a goal of increasing the consumer’s independence, productivity, self‑determination, integration, and inclusion in the community. The majority of time will be spent providing services to youth in the family home, educational setting, program site or other community settings, with minimal time spent at the Center. This position reports directly to the CBS Coordinator or designee.
