Job summary

Sheppard Pratt is seeking a dedicated care coordinator to support the mental health needs of clients in our Baltimore area clinic.

You will coordinate services for assigned clients, develop individualized service plans, and provide ongoing supportive assistance, with on-call flexibility. Schedule is Monday–Friday, 8:30am to 4:30pm, in person.

Qualifications

• High school diploma with 3-5 years of experience or Bachelor’s degree in mental health related field.
• Must have a valid driver’s license with 3 points or less and access to an insured vehicle.
• Role may include on-call hours.

• Coordinating services for assigned clients, including psychiatric and medical care, housing, entitlements, etc.
• Developing individualized service plans with clients based on their goals.

The pay range for this position is $20.60 hourly minimum to $23.69 hourly maximum. Pay for this position is determined on a number of factors, including but not limited to, years and level of related experience.
