Job summary

Wexford County District Health Department seeks a full-time adolescent counselor/therapist to deliver school-based mental health services. The role spans 3 days in Manton and 2 in Brethren schools, with potential summer hour adjustments.

You will assess, plan, implement and evaluate care while collaborating with a multidisciplinary team to support youths and their families. The position requires a Master's degree in social work or counseling and licensure (MSW-Clinical or LPC) preferred, with

Qualifications

• Requires Master's degree in social work, counseling, or related field.
• Fully licensed MSW-Clinical or LPC preferred; Limited Licenses considered.

Responsibilities

• Conduct comprehensive psychosocial assessments for adolescents.
• Develop and monitor treatment plans with multidisciplinary team.
• Provide short-term, goal-oriented counseling addressing identified needs.
• Coordinate mental health counseling/therapy services across care teams.
• Maintain timely documentation of assessments, plans and interventions.
