Job summary

Enhabit Home Health & Hospice is seeking a scheduler to coordinate patient appointments with field clinicians and partners. You will manage the on-call notebook, ensure care continuity, and liaise with patients, physicians, and referrals to support optimal home-based care.

The role requires an LPN license, CPR, and at least one year of clinical experience, plus strong communication and technology skills for on-site coordination.

Qualifications

• High School Diploma or equivalent is required.
• Licensed Practical Nurse (LPN) license is required and must be maintained.
• Valid CPR certification is required.
• Minimum of one year of clinical experience in healthcare.
• Must have and maintain a valid state driver license and automobile liability insurance.
• Basic technology skills including operation of a mobile device.

Responsibilities

• Schedule patients to field clinicians and coordinate care across staff, patients, physicians and referral sources.
• Maintain on-call notebook and hospitalization logs to improve communication among stakeholders.
• Ensure continuity of care and effective supervision and teamwork on site.

Responsibilities

Responsible for scheduling patients to field clinicians. Communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers in order to maintain proper care coordination and continuity of care. Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders

Qualifications

• Required to have High School Diploma or equivalent.
  • Required to be a licensed practical or vocational nurse.
  • Required to have and maintain a valid clinical license, per state requirements to complete this job in accordance with applicable board’s rules.
  • Required to have minimum of one year of clinical experience in a healthcare setting.
  • Required to have and maintain CPR certification, as required by position.or
  • Required to have a minimum of one year of home health, hospice, or pediatric experience within the last 24 months with demonstrated experience and understanding of staffing and scheduling requirements related to home care services.
• Required to have and maintain a valid state driver license in the current state of residence
• Required to have and maintain automobile liability insurance, as required by law
• Required to have basic demonstrated technology skills, including operation of a mobile device.
• Preferred previous experience using Homecare Homebase (HCHB).
• Preferred bilingual (English/Spanish) with the ability to facilitate clear communication with patients and caregivers, improve service delivery, and support positive patient outcomes.
• Must maintain dependable transportation in good working condition and be able to safely drive in all types of weather conditions.
• Oral communication, written communication, fluency in English, active listening.
• Demonstrated intermediate technology skills. Especially Microsoft office, including word and excel.
• Other than for designated full-time remote positions, in-office attendance is essential in order to achieve Enhabit objectives, including but not limited to, effective supervision, teamwork, task completion, and other position objectives.
